Department of Defense Employees (Military, Civilian, Contractors) will submit RFIs/RFAs to the DHA-J33 using the Enterprise Task Management Software Solution (ETMS2).

The DHA Form 435, "Defense Health Agency Operations Center Request for Information/Request for Assistance", is the official method to transmit formal inquiries and requests.

• Click on the form on the right and download the DHA Form 435
  • Fields 1-3 and 11-14 must be filled out
  • Complete Fields 4-10 as applicable
• Submit completed DHA Form 435 using ETMS2 to the Request for Information Team: DHA-RFI_RFA

For those WITHOUT ETMS2 access or if ETMS2 is down*, email the completed DHA Form 435, as an attachment to: dha.ncr.operations-j-3.mbx.rfi-mgr@health.mil

• Fields 1-3 and 11-14 must be filled out

*When ETMS2 connectivity is restored, the requestor will enter the email content into an ETMS2 tasker for visibility and records management
